Information Needed to Connect

The following information will be needed before a connection date can be established:

Residential

  • Applicant's name
  • Applicant's Social Security number
  • Billing/mailing address
    • If different
  • Contact information of relative, not residing in same location
  • Contact phone numbers
  • Date of birth
  • Driver's license, government issued ID, passport, or green card
  • Employer information
  • Proof of purchase, lease agreement for either rental property or lease to own property
  • Residential address for utility service

Commercial

  • Applicant's name
  • Applicant's Social Security number
  • Applicant's title
  • Billing/mailing address
    • If different
  • Business address
  • Business Federal Tax ID number
  • Business name
  • Certificate of Existence from the Georgia Secretary of State (if applicable)
  • Copy of the Business License Authorization form from the Planning & Development Department (if the business is located within the City limits of Lawrenceville)
  • Owner's name and contact information
  • Proof of purchase, lease agreement for either rental property or lease to own property
  • Real estate agent, bank and management company's contact information
